Outpatient Diabetes Self-Management Program
People with diabetes can take charge of their health—and their lives. Norwalk Hospital’s Outpatient Diabetes Self-Management Program has been recognized by the American Diabetes Association for Quality Self-Management Education. Providing the knowledge and skills you need to successfully manage this potentially life-threatening condition, the program is staffed by certified diabetes educators who have been recognized for their experience and expertise. Services are tailored to meet each participant’s individual needs.
Designed for those with type 1, type 2 or gestational diabetes, services include:
One-on-one classes take place Monday through Thursday during the day. Group sessions meet both during the day from 10:00 a.m. to 12:00 p.m. and evening from 6:30 to 8:30 p.m. All program sessions meet at Norwalk Hospital, with language interpretation services available.
Most insurance plans cover diabetes self-management education programs. Norwalk Hospital is also a Medicare certified provider.

Barbara Nadolny, RN, CDE – Certified Diabetes Educator and Registered Nurse
Barbara is the coordinator of the Outpatient Diabetes Self-Management Program and has worked at Norwalk Hospital for more than 20 years. She has 15 years of education experience. She received her Bachelor of Science in Nursing from The College of New Rochelle.
Nancy Held, MS, RD, CDE – Certified Diabetes Educator and Registered Dietitian
Nancy has been working with patients with diabetes for more than 20 years, including 15 at Yale-New Haven Hospital and Yale University School of Medicine. She received her Bachelor of Arts degree from Hamilton College and a Master of Science degree in Human Nutrition from University of Washington.
Pamela Howard, APRN, CDE – Certified Diabetes Educator and Family Nurse Practitioner
Pamela has been educating people with diabetes since 1979. Pam received her Bachelor of Science degree in Nursing from the University of Connecticut, her Master of Science degree in Nursing from the College of New Rochelle and a Post-Masters certificate as a Family Nurse Practitioner from Sacred Heart University.
Nancy Rennert, MD – Chief of Endocrinology and Diabetes, Associate Clinical Professor of Medicine, Yale University School of Medicine
Isabel Valverde – Administrative Assistant
The Outpatient Diabetes Self-Management Program is covered by Medicare and most insurance companies.
